Blood Pressure Connect is a home monitoring program offered to patients by their primary care providers or specialists. The program offers patients and their care providers a way to keep track of their health data and to collaborate on a care plan between office visits.
Developed and supported by the Center for Connected Health, Blood Pressure Connect is a turn-key service for practices to offer home monitoring services for their patients. The Center provides all customer service support.
How does it work?
Using a blood pressure monitor and a phone-based or wireless communications device, patients send their blood pressure readings to a personalized, secure website which allows patients to track trends, provide commentary and review helpful education resources. Providers have access to a dashboard where they can view their patients’ blood pressure data and trends.
Our blood pressure monitoring platform was studied in a large randomized controlled study at a large employer. That study also suggested improved blood pressure control by those enrolled in the program.
The Center continues to evaluate clinical outcomes, as well as patient and provider satisfaction, engagement levels, and determine workflow and product improvements.
