A new program for heart failure patients brought to you by Partners HealthCare.
We are pleased to introduce Connected Cardiac Care, a home telemonitoring program for patients diagnosed with heart failure. Connected Cardiac Care allows you to monitor your health daily and provides a link to your healthcare team from your home. Our Connected Cardiac Care team brings easy to use equipment to your home, installs it for you and shows you how to use it. There is no cost to enroll or for use of the equipment.
Why should I be in Connected Cardiac Care?
Heart failure is a chronic, life changing and difficult disease to manage. In fact it is the number one reason for hospital admission among people over 65 years of age.
This 4 month program will help you and your physician manage your heart failure from your own home. The goals of the program are to help you better understand this disease and be able to make changes to your lifestyle to prevent hospitalizations. At the end of the 4 months you will have the confidence to take charge of your own health.
What actually happens in Connected Cardiac Care?
You will be provided with a small, lightweight monitoring device that plugs into an electric outlet and works by using a standard telephone line.
Every morning you will measure your blood pressure, heart rate, oxygen levels, and weight, and answer a set of questions about how you are feeling. This takes about 10 minutes. Once you have completed the session, your information is then sent to the telemonitoring nurse.
If your readings are normal the nurse will call you every two weeks to talk to you about your condition and answer any questions. The nurse will also call you if your readings are not normal, or if your doctor needs to be contacted about medication adjustments or changes to your care plan. In addition, you will have an 800 number to call the nurse if you have any questions.
Using the monitor, you will be able to look up educational information and learn more about your condition. You will get health tips on diet, exercise and lifestyle changes you can make to improve your health.
I'm interested in the program, what happens next?
The Connected Cardiac Care Coordinator will contact your doctor to set up your plan of care. Once this is done, a nurse will contact you to schedule a visit to your home. We will set up the equipment and teach you how to use it. Every morning, you will take your readings, answer the questions about how you are feeling, and send it to the telemonitoring nurse. The nurse will call you once every two weeks or more often depending on your health.
